I'm PG again after a very difficult instrumental delivery (very large epi, not a natural tear) and the one question that the MW was concerned about was continence. I think that if you've had any problems following your 4th degree tear then it is highly recommended that you have a CS to avoid potentially very serious continence issues due to another VB.
I would ask the usual questions - what are the changes of having another bad tear? If you do tear, what are the chances of serious problems? What could be done about those problems?
There are lots of people on here who have multiple CS though, so it must be posssible to cope with a small child and a CS recovery.

I had a third degree tear after ds 2, now 6 months old, and was in theatre for 3 hours being stitched up as the tear was in such an awkward place - up the bum somewhere of course or is that too much information! After my first consultation a couple of months ago, the consultant told me that providing I continued to heal as well as I was healing, I would only have a 5% chance of tearing again should I give birth naturally.
