I bloody hate it.
Try to relax and let your body do the pushing for you (I think it is called the "expulsion reflex") to give downstairs time to expand. Have also heard that regular perineal massage in the weeks before before giving birth sometimes helps.
same as shesastar some panty type breaths then little push, little push, little push, little push......it stung like an absolute fucker but it gave me focus, head pooped out and was so pleased with myself to then have to go through a lovely shoulder dystocia moment strangely enough everyone was panicking but me at that point, I was the calmest I had been through the whole labour
pooped?!?!?! I of course meant popped , although I may have pooped also, Im not sure
