Lost matb1 form

Does anybody know or have this happen to them before? I have to send my original matb1 form to both my employers, the first I sent it to said they sent it back but I think it's lost as it hasn't arrived in over 2 weeks. I've read I can get a duplicate from the midwife but will this have to be sent to the first employer again as they have specific serial numbers on and both employers would need the same form? Or can the first employer use the one they saw and the second use the duplicate? Thanks in advance.
