Poo phobia - help

I potty trained my twins last summer and one twin has things totally sussed. Twin 2 other has had a poo phobia since getting a tummy bug in the second week of potty training and poo'd herself in quite a bad way.

We resorted back to nappies for a short time, but then tried again. She was absolutely fine but terrified of poo. Since then we have never managed to get her to poo in the loo and all attempts have led to her in a total hysterical mess, crying, screaming and holding it in until she actually poo's herself which leads to more trauma.

I have managed to get her to only poo in the bathroom, only have a nappy for the poo, which then immediately comes off, and am able to sit her on the loo with a full nappy, where I then let the poop fall down the loo. She will look at it 'wave it goodbye' etc, but refuses point blank to poo without a nappy on.

Can anyone help as I am really at my wits end and am out of ideas on how to progress this.
