I'm trying to remember the name of this book? Anyone read it?

It's a bit of a longshot, but here goes.

I really want a book that I read ages ago, but I can't remember the title or author. I'm sure it's quite well known...

...It's set in the 50's or 60's, in England, and centres around an old fashioned mental asylum/hospital. It's about a woman, (who is the wife of a doctor who works there) and how she falls in love with a charismatic patient (he is detailed to work in her garden, or something).

They end up 'escaping' together for a while before they are captured. She is then forced to become a patient in the place that she used to live. There's a sort of evil psychiatrist figure in the background who spends some time as the main narrator and is kind of the protagonist.

Now, be assured, it's much better than it sounds!
