HELP!!! I need to write a statement of fact to dispence with fathers consent

Hey all, my partner and I are starting the process of him adopting my son as the bio father has barely been about (left me @4 weeks pregnant, and only seen DS a handfull of times in 1st 6 months then no conact since)

My Partner and I got to gether before i gave birth and IS daddy - a blooming great one at that! anyway wafflign abit....

As I do not know the whereabouts of my DS bio father (have taken various steps to gain contact for consent) we will need to ask the court to dispense of his consent (as he is on the birth cert - he was about at the time and he is who he is!) and write a statement of fact,........its this i have no idea where to start

Has anyone else written a statement of fact to dispence of a parents consent and able to pass on any tips on general format & what to put in it (without totally slating the waste of space as im sure that won't look too good!)

Any advise & guidance would be GREATLY appreciated.
