I did find one but it had 2008 all over it.

I'm looking for the one that you download to your computer to fill in and then print off.

Do any of you nice helpful ladies know where the current one is?

no sorry no idea but if its a first application youre better off ringing and getting one sent as then they can backdate to the date you phoned them!! Sorry i know this doesnt answer your question though!!

The online ones dont give nearly enough room/space/questions for the indecision maker to make no decision, as jenny says better to ring and get a paper one.

Add extra sheets to that too, make sure they have plenty of info to base their crappy decision.

And if you were wondering, im not at all frustrated that they have now been sitting on 2 of my DLA renewals for 12wks and 8wks. , and have just recently written to school for reports that are obviously not even going to be looked at until mid september!

Thanks - Sorry, but it's a renewal & I was hoping to make it easier by just doing some copy and paste from the first one.

Another question is, for an ASD child, which professionals did you get to support the application? Most seem to drift away after dx.

we struggled too, we put: HV, portage worker, erm cant remember. I know that they are asking for reports from the world and his wife! OT, school, Paed, its very frustrating.

I think ANYONE who is a 'professional' and KNOWS the child will do.

It's bloomin' early for them to send us the paper forms (which they have) - Renewal isn't due for months yet (nearly 6 months away).

average is taking 4mths at the moment. They give them to you so far in advance so that they can sit on them for 3mths...

if you do let it slip until last minute your payments will stop. And worse perhaps if you have to appeal a decision it will leave you in limbo land without payments until its sorted. I know its horrid, but grit your teeth, and get it started. did you keeep a copy of the last one you sent?
