Child’s plan (Scotland)

My youngest son (age 3) is going through assessments for ASD, his behaviour is now very much controlled at home since we identified his triggers and started SALT.
He struggles socially at nursery and my health visitor has mentioned about coming to my house to make a “child’s plan”

There is absolutely no social work involved in my family and no welfare concerns as regards to our parenting skills. So I just wondered what would appear on this “child’s plan?”

Is it in preparation for educational needs at preschool and if so does anyone have any experience of this?

I know that my views will appear in the plan too and just want to ensure that he receives the best support available.
