I've just received these for DS in advance of an appointment. On one of these (CAST) they are asking for yes or no answers. I'm finding this very difficult for some of the questions and want to elaborate or say "sometimes" or "depends". For example, it asks if he joins in easily with other children. The answer is it depends! if it's an activity he enjoys such as something creative or computer based he will absolutely join in. However, if it was football or some other noisy team game he wouldn't be interested. Is it OK to indicate on the form that I can't give a definitive answer to those questions? It's really not black & white with him!

I would write on the form or on a separate sheet of paper Smile

When DS's teacher filled if some of these forms she added loads of comments in small writing at tech side

I added lots of free text when completing DS's questionnaires.
