ADHD or not?

DD is 8 and we have concerns she may have ADD or ADHD. These are long founded concerns since she was a baby. As a yound child she never played with any toy for any length of time, was constantly flitting between activities and needed constant attenton. As she has got older she talks excessively at school she is always in trouble for talking. Sleeping has always been an issue as a young child she would be up several times a night and would take 2-3 hours to get her to settle despite trying all the usual techniques. Now she does sleep bettter but still usually takes over an hour for her to settle sometimes 2 and she still gets up in the night sometimes. At school they say she has a number of inattentive behaviours - getting out of her seat frequently, needing to be redirected to her work, constantly losing stationery, clothing etc. Her recent OT report says she observed distracted behaviours at school and difficulty with attention in class.

Would this be indicative for ADHD or ADD?

We have raised our concerns before with Paed but she has never been assessed for ADHD/ADD but it is noted on several reports that she has issues with her attention.
