Kerry Farm are based in mid/north Wales and are offering holidays to families of disabled children for a week's short break. The facilities are fully accessible and there is no charge for the accommodation (self catering).
From looking on the site it seems they are also able to provide support, respite and activities during the holiday too as well as travel expenses.
It's not means-tested so a lot of families can apply.
