So we applied for DD to have SA. They agreed to it first time (could've knocked me down with a feather!)...

So I'm filling in my Form 2. Could someone read it through for me and tell me what needs to change, please? Thanks!

If it helps, with Form 1 I submitted:
Paed A's report (since retired)
Physio report
Playgroup report

They have since had an Ed Psych report.

I'm now enclosing:
OT report
Paed B's report (new paed)
