I have to do one of these for my brother - who is middle aged, has cerebral palsy and profound earning disabilities. Still, I am sure there are employers lining up to offer him the support he needs to be able to work!

Anyway, fury at political attacks on benefits aside, does anyone know of a guide (a bit like the Cerebra one for DLA) that I can follow.

The questions seem quite similar in that they are clearly intended to elicit short answers but require far more complex information.

That should of course be profound LEARNING disabilities. But earning disabilities has a ring to it !

Is this the medical form they send if you are on/applying for Employment and Support Allowance, from which they decide if you have to go to ATOS for a face to face assessment? If so, I did one for DD. I just copied things I had put on her DLA application - which I had only just renewed at the time as she had turned 16. She didn't get called for a Work Capability Assessment which is a shame as DH wanted to take a day off to take her and see what they thought she was capable of doing
