Are there any example Parent Advice for SA on the internet?

Hi
I am trying desperately to fill in the Parental Advice form for the LEA who have agreed to carry out a SA of my son.
I seem to keep going round in circles, rewriting, taking bits out and generally tripping over myself!
I have googled but can't seem to find any example ones on the Internet. Are there any out there or have I really got to do this blind?
I thought the DLA forms were bad but this feels much worse - more riding on it somehow.
Thanks for any advice.

Hi Annie - I have been googling for exactly the same thing this week and like you have come up with nothing. It would be SO helpful to see an example of a case's submissions even though every case is wildly different.
TBH I'm just struggling on getting the request done - desperately want to do it before christmas. Did you do a simple but concise request or did you throw everything into it ?

i dont have any advice to be honest either but am happy to email anyone mine if its any help, my ds was 4 at the time and in a mainstream nursery we wanted special school and we got it. he has autism and probably adhd and challenging behaviours a plenty :)

Hello,

I would also be grateful to read them if thats ok about to begin the statement battle, dd 3.5 dx autism due to start school next Sept have been told by the head only children with physical disabilities can get statements nowadays hmmmm.
