Just that really. Is there a ceiling to the level/sub-level that a child can get?

Yes, 4a on the actual year 4 tests.
However they can be assessed as being higher. I have some children working at Level 5 in my class. They will do a Year 6 paper when the others are going the Year 4 papers.

The reading and writing papers just have Level 4 as the maximum, with no sub levels given. It is only the maths paper that allows a 4A to be given.
