LLETZ treatment - advice needed [edited by MNHQ]

Hi everyone

Recently got my BFP for DC2. Very early days but very excited and spoke to my GP surgery via the phone who advised me to come in and fill a form out which would be passed to the midwifery team, and unless there were no medical reasons for an appointment I didn't need to go to see my GP.

What I'm not clear about is this - I had LLETZ treatment in 2015 for CIN1. Is this something I need to see the GP about or make known before my booking in appointment? I'm assuming not, but would appreciate some advice.

Would also appreciate any advice or stories from ladies who've been through the same and what this meant in terms of your pregnancy going forward; does it mean any different care/treatment/additional monitoring?

Every trust is different. Some trusts only monitor if you've had lletz two times or more. I had lletz for CIN2 and I was referred to a consultant to check cervical length. For me it turned out that I had a soft cervix and it was also shortening, so I had weekly appointments until 28 weeks, then fortnightly and was also put on progesterone pessaries. Mine may have been totally unrelated to the surgery, and my cervix could just be a bit shit. My cousin had CIN3 and fell pregnant three months after the surgery, she went to term and was monitored just in case. But her next pregnancy she went unchecked and wasn't due to see the consultant until 33 weeks, she had her DS at 32 weeks. He's fine, but the consultant apologised for not seeing her sooner. For me, I would personally insist on a scan to check cervical length, but that's down to my experience.

Thank you so much. I had the treatment at a pretty rough time in my life and I'd put it to the back of my mind. I can't remember the ins and outs of what was said at the time but do recall being told to mention it if I were to conceive in future. When you say weekly appointments - is that weekly internal examinations? What did they entail? Have some anxiety around that due to an assault when I was younger; I'd just rather know what I have to come so I can be mentally prepared.

I told the Midwife at my booking appt and she referred me to a consultant. I didn't get seen until about 20 weeks at which point the consultant looked at my notes and said if you were going to have a problem, you would have had one by now. Which was pretty awful! So I would definitely chase up to be seen asap just for peace of mind. I didn't have any problems at all but the point of being referred is to hopefully prevent any problems before they occur!

My midwife asked about any treatment I had like that at my booking in appointment, I had had LLETZ treatment and something else in the past so I was put on to consultant led care. Saw a consultant about 16 weeks who looked over my notes from the treatment and said I would be fine. From what he told me if you had 2 or more LLETZ they need to monitor you more often. I was referred straight back onto midwife led care at that point.
I had no internal examinations over it. I hope for you it is just as straightforward as it was for me

I had a LLETZ for CIN 2/3 and told my midwife at booking in who referred me to obstetrician for review. You don't need to tell your GP, just midwife. The obstetrician will look at how much tissue was removed by accessing the records. If it's less than 10mm they aren't too worried but if it's more they will send you for a cervical length scan. I had 8mm removed and they scanned me anyway and all was fine. My cervix really long!

I had two LLETZ treatments around 2yrs beofe pregnancy. It wasn't considered a problem during pregnancy, and not mentioned after booking in. But turned out when I went to give birth my cervix wouldn't dilate and had to have an EMCS. It was a near miss apparently.

We should have realised my cervix wasn't right tbh as I'd had problems with my periods not flowing out following LLETZ. In my post pregnancy colposcopy my consultant said it was rare to get cervical stenosis to such a degree.

Second dc was a welcome planned cs.

I had LLETZ in my early 20's. I was pregnant last year and mentioned it to the midwife at booking in. She didn't really seem to register it and I thought it wouldn't be a problem. Luckily, I also had Hyperemesis and was taking part in a study regarding that meaning I had more regular scans. It was during a 14 week scan that I mentioned it to the professor conducting the study who looked at my notes and found the midwife had failed to record it.

My cervix was measured there and then by the consultant who happened to be passing in the corridor outside and found to be borderline insufficient.

I was booked in the next week for a cerclage stitch. After that I was scanned very few weeks to check for funneling and that the stitch was still in place.

I delivered by c-section at 38 weeks (he was breech and had refused to turn by ECV - whole other story). The stitch was taken out at the same time.....well not completely true....High as a kite on the drugs as they wheeled me out of theatre I said "And I didn't even feel you take the stitch out!". Cue surgical team all looking at each other saying: "did you take it out", "I didn't take it out - thought you did" and me getting wheeled 180 degrees back into the operating room.

I would just make sure everyone is aware, mention it at every appointment, don't expect them to know from your notes and, most importantly insist on a cervical length scan.

The regular scans were internal yes, they do an internal scan to check the cervical length. Really sorry about the assault as you can see here, everyone has a different experience. I'd had some bleeding following intercourse prior to 12 weeks and after too, I had bought this up with the consultant when I was there for something unrelated to my lletz and received the letter inviting me to have my cervix checked a few weeks later.

You need to be monitored < 3xpprom, 2xpremature babies, 2x cervical sutures, tac&tvc - failed, 7mc including one 17w>

Don't let them fob you off.

I’ve had lletz and had 2 kids fine but you do need monitoring

I’ve had the procedure and have 3 children with no issues. In my first two pregnancies I was put under consultant care as a result and had two weekly TV (internal) scans to check cervical length for part of the pregnancy. I think it was after the 12 week scan until after a short time after the 20 week one. I would make sure to mention it.

I told my midwife at booking and was referred to a consultant. Had scans to check scarring and see if there needed to be additional stitch to help with strengthening of uterus but I was ok. Good luck x
