MAT B1 form

Hi all ,
I've just had my 20 week scan and I completely forgot to ask for this form. My next midwife appointment is at 28 weeks and need this to show my employer.

Has anyone obtained one from their GP? How easy is it to do and do I have to do anything in preparation?

The Mat B1 forms should come from your midwife. Could you call the midwife and she can leave it somewhere for it to be collected?

Yes you can get it from the GP, just phone and ask.

Just ring your surgery as request that they get the midwife to do it for you and leave it at reception for you to collect. That's what I did.

The GP can't do it as it needs to come from your midwife but you don't have to physically see the midwife to get it if that makes sense.

Make sure it's on your to do list.
I didn't get mine and had ds at 35 weeks. Caused all sorts of problems as I hadn't had it signed. .

I had this exact scenario a few days ago- I called my midwife and she's filled the form in and left it for me to collect at the GP, maybe try that? X

Thank you everyone , I will call my midwife as my GP surgery is in a different borough to my chosen hospital and am unsure as to how this might affect me obtaining one from there. It's is very confusing!

I was so relieved to get good news at my 20 week scan (last pregnancy ended due to bad news at anomaly scan) that I skipped out of there without asking for the bloody form!

I had to go to the hospital reception desk with my hospital notes and anomaly scan report, they then got a random midwife to collect my paperwork. Might be working checking with the hospital reception as it might be something you can just pop in for.

You can't get a matb1 form until you are 21 weeks anyway so wouldn't have been able to get one at your scan.

I would just try and book to see your mw in the next couple of weeks to get one.
