Reason of crippling condition in new born babies

Recently, I have come across a case of Sommers & Roth A baby born with no hands or feet due to the failure happened in reading and interpreting ultrasound. The technologist and doctor's report stated that there are no abnormalities during the ultrasound performed on the 17 weeks gestation, but the baby born with a crippling condition. I would like to know what is the reason of crippling happened to the child? What deficiency prevented him to being born as a normal child?
