... I have just read I should have had an exemption form for prescriptions etc. Is there anything else I should have had? I will ring next week and wanted to double check there wasn't anything else I was missing!

You can ask your GP surgery for a maternity exemption form, in some places you get a Bounty pack too but you're not missing anything if you didn't get one. At about 20 weeks you should get your MatB1 if you need one for your employer.

A booklet on screening tests ( although my midwife gave me the wrong one!)
Less essential- Emma's diary, shed loads of leaflets advertising stuff

I got mine at booking and a Bounty Pack but friends at different hospitals have had to ask their GP for the exemption form. You will probably have a routine GP appointment soon anyway so just ask them then.

Your GP arranges the form - but ring and chase if you don't get one.

Many chemists will accept seeing your notes as proof of pregnancy while the form is in the post.

I was never asked for a maternity exemption form - they took my word for it!

I just tick the box. No one has ever asked for my card.

Mind you I am glad I have it in case I'm ever asked!

You wouldn't normally get your MATB1 until 20 weeks. And prescription form from your GP. Sounds normal to me, all I got was my notes (and Bounty bumpf) both times.

Congratulations :)

At my booking in I was given notes, a bounty pack (loads of advertising with a sample or two) and my midwife took some details to register me with the hospital for my 12 week scan appointment. The confirmation cane by post.

FYI, at some point you will be asked whether you have a stable home life, real life support and whether you have experienced domestic violence. I asked around at the time I was asked and this is standard procedure apparently.

I asked my GP about this when I was 6 weeks pregnant. He didn't know what I was on about! He told me to ask the midwife at my booking appointment. I've got that appointment in 3 weeks. I'll be about 11 weeks then. That's the earliest I could be seen. Hope I don't have to wait ages for the scan.

I got my notes at all my first appointments and the rest of the bumpf at the 12 week scan.

I never had one. There's a box to tick on the prescription to say you are exempt and the pharmacist ticks a box to say they haven't seen proof.

Same applies for free NHS dentistry.

I didn't get given any bounty bumpf - I wonder if this means my NHS trust have moved away from bounty? I kind of hope so after my experience on the ward with the bounty photographer with DS ...

I got a bounty pack at first mw appt. The receptionist at gp gave me form which i completed and handed back and got a card through post for free prescriptions and dentistry etc

Bounty pack was jst full of advertising anyway its handy to hold my notes though and photos but thats about it.

I took the pack the midwife gave me but all I got out of it was a sample of stretch mark cream! I didn't bother with the big Bounty pack as I don't approve of their presence on maternity wards and didn't want to be on a mailing list forever!

I don't believe the free prescriptions form exists! When I first went to GP, didn't occur to me to ask until I went to pharmacy to fulfill my folic acid prescription. -I then had to go back to GP (2 doors down, not much faff!) cos pharmacist queried the amount prescribed, asked about the form, was told to ask at first midwife appointment.

At first community midwife appointment she said she'd leave it to collect with my notes - nope!

At hospital midwife appointment she just said GP can get one but just wave your green notes at pharmacists and dentists instead!

Ref Bounty packs - completely agree with Cariad, their presence in our NHS is obscene - however having been handed it, took it home anyway and going through it got 6 or 7 samples a various things, a few money off vouchers and 2 carrier bags of bumf to go in the recycling.
