fatigue and memory in pregnancy survey

Hiya,
I'm an undergrad psych student @ Newport uni. I am researching memory and fatigue in pregnancy. If you'd like to help me, it takes 2 mins. Informed consent - the survey is anon and fully confidential. You can withdraw at anytime.

I've done it. Fwiw, the survey has some "holes" in it.

I ticked yes to "3. Did you suffer a pregnancy related health issue?" but then my main issues - SPD and OC - weren't listed.

On Q "5. If you suffered fatigue when pregnant, how severe do you feel it was?" there is a grid of answer options, but with no explanation of what they all are (think it should only have 5 options of Very to Not at all and no grid.

Same issue on Qs 8, 11, 13, & 14.

On Q "9. Did you suffer fatigue during any previous pregnancy?" there is no option to say "No previous pregnancy".
