They do spend more than you think on healthcare, it just doesn't cover the whole population. It is an expensive system- costs are much higher.
I don't have the figures on me but their public spend is not too far off other countries, they have the private spend on top.

Without looking it up, I believe the US's biggest expenditure is on defence.
