Can someone help me with this complaint letter please.

I am getting stuck and in a muddle. I suspect that i am too emotionally involved at mo to be coherant so could some one manipulate this to make sense. TIA.

I am writing to complain about the misuse of the CAF document concerning my son X. It was my understanding when the CAF was requested by his GP that it would enable a meeting between all of the dicsiplinaries involved in his care to come together to discuss the situation and move it forwards and to share relevant information between the parties.

Recently there was a problem with an appointment for a peadtrician which had to be changed. In this instance my ex?s mother was rung with the information. When I rang to find out where I stood with regard to the meeting I had the information on the hospital system checked and found that the contact details were correct. The only reason that I could come up was thtat the CAF document had been used to extract the contact information and the number miss read as the number rung was that where my son?s father resided.

This issue has been further compounded by a letter detailing a recent visit regarding a previous head injury. I was distressed to find out that not only was this letter being sent to me but also being forwarded to individuals who were invited to the CAF and but have not been involved in his care since prior to the CAF and to his preschool which he has now left. Neither of these parties require the information and I feel that it is a serious breach of not only patient confidentiality and the data protection act which covers the usage and disclosure of the information stored electronically. I am aware you cannot retrive the mailings but am concern about the usuage of the CAF documentation and very unhappy about the situation?.

Well, there are a couple of spelling errors:

Paediatrician
misread
retrieve
usage

I'd also slightly change the third para to read "..serious breach not only of patient confidentiality but also the Data Protection Act which covers...."

Other than that, you letter is clear about what went wrong. Maybe you should say what you want them to do about it, eg a meeting or telephone meeting, and when that will take place.

I checked with the hospital that my contact details were indeed correct. The only explanation I am left with is that the CAF document has been used as a source of contact information and that somebody mistakenly used the number of the residence where my son?s father resided.

Right another question: who would you send this to? the pead, the hospital complaints dep, both of these, the CAF controller (nominated starter individual), the individual who will be in controll of the CAF as of sept or both, everyone above? Its a very complicated thing.

Em, I would ask elsewhere about that, is it a special needs issue? Maybe worth trying on that board.

I'm not too bad at grammar but no idea what a CAF is, let alone where to send your letter, sorry!

Actually not an SN things its a new form thingy that is being put in place when a child has to see more then one specialist and has a need to have across board meetings. Take point and will try there though as they are good at this sort of stuff.

Sorry, didn't mean to offend! I really have never heard of it so it was just a guess because there were so many medical people involved.

Its ok no offence taken. Just never thought it would be appropriate there as wasnt really a SN issue. But i have already been given some good advice. Thanks
