Writing a letter - help!

I don't know if i am posting this in the right section, but i figured this would be where i would get the most correct help (for want of a better word).

I want to write a letter to someone who is in her late 40's and quite 'proper'

It is an apologetic letter for my behaviour some years ago, i could let it lie, but i feel unsettled whenever i think about it and would like to send an apology.

Firstly, how should begin the letter? Also how should i end it?

I am now married, how should i write my maiden name/married name, and how should the layout be?

Thank you very much for any help, it really would be greatly appreciated

Dear Fredwina,

Yours sincerely,

Mary Brown (nee White).

Layout should be just a few brief paragraphs, dealing firstly with why you've decided to write, what exactly you're apologising for and how you hope that she will accept your apology.

Head your letter in the right corner with your address & the date.

That's the easy bit!
