Medical miscarriage - is this normal?

Hi everyone,
Took misoprostol pessaries this morning (missed mc - nearly 13 weeks by my dates but empty sac at 8 weeks). Was expecting them to work within 4 hours but nothing happened. Then eventually got pretty severe cramps but no bleeding. After 9 hours I had the beginnings of bleeding but then the pessaries came out whole - I definitely followed the instructions properly and put them up as far as I could, are they supposed to dissolve completely? 9 hours seems like a really long time for them to have been in and not dissolved at all. They were quite soft and I put them back in about an hour ago but still nothing new. Just wondering if anyone else has ever had similar?

My EPU said to ring after 48 hours if doesn't seem to have worked and they give you a second dose but I'm not sure I can go through the cramps again if it doesn't work a second time, I would probably just go for the d&c (it was going to be 2 weeks before they could book me in so went for medical management as it's taken them 5 weeks to officially diagnose miscarriage and I'm just emotionally done)

So sorry you're going through this, Darcey. Did anything happen overnight?

I don't have any experience of vaginal misoprostol as I've only had it orally. It took about 5 hours to work.

If I were you, unless it's changed overnight, I'd give the EPAU a call. They may be able to get you in for surgery as an emergency referral.

Thank you. Bit of bleeding but only like a light period really. Cramps have stopped. Definitely not passed the sac or any clots. Rang EPU but they said to ring back tomorrow as they don't do anything before 48 hours have passed. I'll call tomorrow, hopefully can get d&c done quickly - don't want to wait for ages longer - I think the last 5 weeks have been enough!

I had the pessaries put in and after 48hrs was still just bleeding heavily, called epu who didn't offer to provide more just booked in a scan for 5 days time :/ Literally the day before the scan I passed the sac and baby. Hardly any pain passing it but was extremely distressing. So it took a week of cramps and heavy bleeding and then the passing.

I am probably in the minority knowing my bloody luck as most people this route does seem to work quickly. I guess actually mine would be classed as failed medical management as it definitely didn't excacebate any bleeding or cramps as I was already bleeding lots before procedure.
Bleeding tailed off shortly after and had a scan a week ago which showed nothing left. Test has gone negative today.

Thanks so much. I went into the gynae ward today and got given a second lot of 4 pessaries. They put 2 in at first then changed their minds and put an extra 2 in then sent me straight home to wait. They said vaginal misoprostol should work more effectively than oral, in theory. It's been over 7 hours now and no cramps, no bleeding. I think this second lot might also not work. They said they can repeat treatment with pessaries up to 4 times but with a 48 hour wait between each dose I don't fancy that. I think if by the time another 48 hours have gone by I haven't passed the sac I'll opt for surgical management. I was hoping to avoid it but I can't just keep being tied to my bed/sofa for days on end waiting for something to happen.

The nurses did say it's quite normal for the pessaries to come out whole, by the way - if anyone reads this and wonders!

I had the 4 pessaries too, not sure why they didn't repeat mine, maybe it's because I was bleeding so much, that they presumed something was happening, but in theory I was bleeding like that and passing huge clots before the pessary. Not much help I know just my experience!
If I was you I too would push for the surgical, it's all just so drawn out and traumatic. I couldn't have the D&C as whilst miscarrying, I also had pneumonia so wasnt at all in a fit state for the anaesthetic.
Saying that I did find it comforting to be able to 'do' something with my baby once passed. I know it's not what everyone would want but we buried the baby in a little box in a huge pot with a rose on top. Just was a comfort to me and my husband.
