Having some major problems with ex. Does anyone know whether he could potentially report my DS passport as lost or stolen and get a new one reissued to him without my knowledge? If this is possible, how can I find out if he has done that?

Contact passport office and inform them of this they can put a note on file.

Yes he could, does he have the passport number & details?
I have this concern at the moment too, I rang the passport office and was told they won't put a note on the file; ex has PR so he's got the right to cancel the passport and apply for a new one for my DC. Not the answer I was hoping for!
Only way to know if a passport is still valid is to take it to your nearest passport office and they check, they can't or won't do it over the phone.
Could you secretly cancel the passport yourself and apply for a new one? That way he wouldn't have any of the passport details so would find it hard to cancel it surely?

Why can't he have the passport, ? Are you fearful of abduction or does he want to take cd on holiday ?

I had to cancel mine cause ex refused to hand them over, then received an email sayin he's lost them. I applied for new. It then hit me he to could cancel so I rang the passport office and they told me the only person who could cancel them was the person who applied, which was me for the first lot and the second lot. I also asked for a note to be put on file as there was a court order in place and residence was with me but they refused.
