Will you spare a few minutes to complete an online survey to help children and parents at both Great Ormond Street Children?s Hospital and Guy?s & St Thomas? Hospital? And to spread the word to other parents too?

The survey is part of the hospitals? HealthTracker ® project which helps children with severe chronic illnesses with interactive online games and also surveys for children and their carers.

To make the survey information more meaningful we want to compare it to survey results from parents / carers of typically-developing children (those without chronic illnesses).

If you have childen aged 5-17 years old and can find the time to take part...here's how to do it - and perhaps win one of four laptop PCs for doing so.

By taking part you will be entered in a prize draw to win one of four HP 530 Notebook PCs.

If you can also spread the word amongst your own parenting network - perhaps the parents email list from school, or else your family and friends
