Help and advice on bottle feeding

Hi,
My baby is 4 months old now, I have been exclusively breast feeding her for over 3 months. I use to give her a bottle of formula when she was a newborn every evening for about 8 weeks but one night she just decided she didn't want to take the bottle anymore and went crazy at the sight of it. I was happy to continue breast feeding as my milk had come in and I could tell she was getting enough but now I'd like to start introducing a bottle again as I will be looking to send her to nursery in the new year when I have to go back to work.
I'd really appreciate some advice on how I might get my baby to accept the bottle again, has anyone else had the same problem? I've tried a few different bottles and teats but nothing seems to be working.
