Child benefit form completion

Hi Everyone - my first time here....:-)
Can anyone help me please I've been on the government website all morning after printing off the above form trying to find out if you put a tick in the box's or a cross to no avail! As an ex civil servant I know how picky some public servants can be (not me I hasten to add) has anyone recently cilled in the form and is now receiving the benefit and can answer my question. Thanks so much Chris

I had a similar issue completing the mat allowance form, phoned the office it was to be sent to and they said they didn't care as long as it was clear.
