Financial Consent Order Form A Q

Ex is applicant in divorce and wants consent order for clean break. This was drafted, signed and returned to her solicitor. OH is LIP.
Her solicitor has now said OH needs to do a Form A but this appears to be for applicant and he is respondent. Assume this is for dismissal purposes only from googling as there is no lump sum, SM, pension sharing etc but nothing is clear.

Does anyone know how to complete a Form A as respondent? There isn't even anywhere for a respondents signature.

Only one party needs to lodge a Form A for dismissal purposes only, and that person IS the Applicant, which is why there is no room for the Respondent to sign. Both parties need to sign the actual draft Order and financial statement, but not the Form A....sounds a bit odd?
