Help with first doctors appointment

I was diagnosed with PCOS at 16 (following transvaginal scan) went on the pill until January aged 33. Four years ago at a scan I was told "there were no sign of any cysts". Hurrah I thought!

Took 74 days post pill for my first period. Then 56, 40, 40, 38 and I'm on cd45. Previously used OPKs and had period 12 days post positive. (I'm aware they're not reliable with pcos)

Last cycle I had dark tar like discharge for 5 days, before a very light bleed for 6 so booked the doctors. Thinking low progesterone.

This cycle it's longer. Day 45, I'm 20dpo. We dtd on the positive OPK day. Egg white cm recorded two days before positive OPK. Negative tests. Dreadful backache on and off for a week.

Should I ask for a blood test on my hormone levels? Ideally I'd have a scan as I don't know if I do have pcos or not now after my last scan. I would say I do. VERY thin hair at front/ extra hair under chin 🤦🏻‍♀️ I don't know what I'm doing really. Can you help a lady and advise? Thank you lots
