Equal Opps Monitoring Forms

I have been asked to fill in an equal opps monitoring form recently as part of a job application however, instead of it being anonymous (as they normally are), they want me to put my name on it. Is this allowed? They have stated that the monitoring only takes place at shortlist and interview stage then once an appointment is made, the data is stored anoymously then the forms destroyed.
All seems a little complicated! TIA

Nothing wrong with them asking for you to put your name on it, although it would be more usual to be something that is part of an application form that is then detached before shortlisting so that your answers can't be viewed by someone doing the shortlisting.

If you're not comfortable putting your name on it, you could refuse to fill it in, or you could ask for assurances that the form is kept separate from your application so that your answers aren't part of the shortlisting process.

Thanks flowery, it doesn't really bother me but thought it was a bit weird from the shortlisting side as I suppose in the wrong hands it could be used to discriminate.
