mention how much you enjoyed the job
you dont need to give a reason why you left, just put something like.........Due to unforseen circumstance I reluctently had to leave my employment.

I am assuming it is an actual vacancy rather than a speculative approach?

I would do a normal application letter, and ring your ex-manager or whoever and have a chat about it before you send it. That way the application is formal and 'proper' and couldn't be seen by anyone to be expecting favours or whatever, but you are testing the ground and making more personal contact at the same time.

Formal application letter, say 'further to your advert in x publication on x date (or however job was advertised) I would very much like to be considered for this post.

As you will note from my cv I have x years' experience of y and z, including x years' with [name of organisation] in this role.

Blah blah blah

Phone call to ex manager or whoever 'Hello [name], how are you? . Anyway, the reason I was ringing was to have a chat about the vacancy you are advertising. I am very interested in returning to work with you in this role, would like to find out a bit more and also just let you know in advance that I will be applying formally'.

Ta da!

Good luck
