back to work after illness

Hi everyone first time posting so a little apprehensive.
I have been off work on a GP note since before easter hols.(i work in a school) i am under the hospital for treatment aswell. my note is due to run out next friday so i was planning on going to work for the last week of term however the GP said full days MON-Fri would be too much and asked me to request a reduced hours for that one week to do just mornings, school said they would get back to me. i rec;d a call the other day saying they are refering me to OCC Health before agreeing to this. Now a bit of back story i have been suffering since Aug last year but battled on in work till easter i asked to go to Occ health so they could put a plan in place for me to stay in work if possible. my line manager said we dont use occ helath never had to so there wont be any need just get a sick note if you need one. i have also had to fill a formal request form in to reduce my hours in september by dropping one day as this would aid my ill health. i have been unhappy at work for some time now and other people have done staggered returns and long term illness never had a problem. but now its my turn and they are making things very difficult. everything i have asked for over the years has been denied for one reason or other ive been promised things that have never happened its all got a bit messy really. what im looking for is a bit of advice on what to expect at Occ Health and also advice on next steps at school when i go back.
