Painful Anal Fissure, scared to go toilet!

Hi guys hope you dont mind the thread, I know it's a tricky subject but would love some advice.
Due to medication and not drinking enough water I tend to get constipated now and then. This has caused anal fissures which I usually deal with fine.

As of 4 days ago it got really bad, the pain is excruciating. But now I'm scared of having a BM, because whenever I try I'm in a lot of pain. I know I need to go, but when I try I have to stop because of the pain.

I started taking Senna, but because I've never taken them before I only took 1 tablet instead of 2. That's done nothing obviously. So I've moved onto stool softners, which I've been taking for 2 days now (i understand it takes a few days to work)
I've increased my water drinking by alot, and I've been using germaloid cream to try heal and help the fissure. I've even bought Vaseline to try and help things.
I know I'm no longer constipated, but I stop myself because of the pain and I dont know what else to do. It's near impossible to get a doctors appointment because of covid, and I dont see this as important when doctors are dealing with the virus.

I've bought a enema (just one you fill with warm water and do yourself) which I was planning on trying tomorrow for the first time. I've researched it and seems it may help clear me out so I can heal. But because of the pain I'm in, will an enema make things worse?

I was going to try the enema, and if that didnt work go onto suppositories. Then go doctors as a last resort. The lidocaine cream does not numb the pain when I try and go toilet. I feel so down over this, even though its only been 4 days. I'm so scared of the pain I'm not allowing myself to have a BM which isnt healthy.

Sorry for the long post, was just hoping for some advice on the least painful way to get through this. Thankyou for any help!

I had a fissure from extreme dihorrea due to medication. It was so painful. I understand how you are feeling. I bit the bullet and went to the GP and was prescribed an ointment called glyceryl titrite which promotes blood flow to the area. You get a bit of a head woosh due to blood flow but only for a few mins. It's really good and helped heal it in less than a week.

Where do you live? I've not found it hard to get telephone appointments at all with my GP. I'm sure if you said you had an anal fissure they would give you some ointment it's not ideal they can't examine you but it's a common medical problem.

I live in a small town that has only 3 doctors and over 4000 patients. I may tr just getting a phone appointment.
I have read about the ointment, but worry as I suffer with migraines and apparently this can cause migraines?

I just want the pain numbed so I can go. And nothing seems to be helping, I usually have a high pain threshold, but I just cant sit through this Sad

Unfortunately the ointment does cause migraines but it does work to heal the fissure. So it depends on whether you can cope with the migraine. Mine took longer to heal than it should as I ended up only using it once a day at bedtime so I could go straight to sleep. Do try and get in touch with your GP

Microlax should be good. I gave suffered from this from chronic could conspitation in the end I had a minor surgery to remove scar tissue and have Botox in the inner anus area. It helped but there were side effects. Microlax safe for most people to use. But speak to Dr or dietician about keeping soft and regular to allow for healing. If it doesn't try asking about what u I had to get done in the end. Good luck x

Get some Anusol cream. It has a spout sort of thing about an inch long, but is very thin, which you gently push inside of you and then squeeze cream inside. It help heal very quickly. You will feel immediate relief. Healing only took me two or three days.

Just keep drinking lots of water and fresh fruit juice and veg.

I had a fissure through having an undiagnosed thyroid condition.

I would say it's a headache certainly not a migraine. So it's not causing migraines in every one. I tried to heal mine on my own and it just didn't happen sorry.

I had a long term one. After many many visits to the doctors this is how I cured it.

I gave up all greasy food, coffee, and spicy food. I used sudocrem after every movement and I used witch hazel wipes after every movement. I used a small dose of lactulose, and Epsom salts baths religiously.

Then I found out about too tight pelvic muscles and that I was. a clencher, I did bits of yoga and exercised for high tone pelvic muscles. I am fissure free now. I also cut bread and constipating things out of my diet.

I don't mind the migraines/headache if it will heal this. I will be taking stool softeners daily while i am on medication to prevent this happening again. But right now i need to find the courage to not give up with the pain and just push through it.

Has anyone had experience with enemas? If i do it tomorrow would it perhaps soften stool enough to help me pass? I just need one BM and i feel like i'll be much better. I guess i'm just worried something serious will happen like i will tear myself or die from blood loss. I know it seems extreme, but years ago i had health anxiety and i feel like it has come back with this.

Maybe if i can't clear it tomorrow i'll call the doctors and see if they can help. I have Germaloids cream which has Lidocaine in and is supposed to numb inside and help with Fissure pain, but it isn't numbing anything.
I know 4 days isn't bad compared to some stories i have seen on here, but it's bad enough for me, i don't want to be scared of going to the toilet for the rest of my life. I'm 29, so hopefully i can push through this fear and then keep a healthier lifestyle to help in the future.

It was much stronger than germolene. The first time I used it I didn't put gloves on and my hand went numb!

Oh wow! If that can help with the pain i know it would be a big help. May give my doctors a call tomorrow instead and see if they can help

It didn't hurt at all once I used the right cream. I thought it still would, but it didn't hurt at all.

Bit of an update, i managed to have a BM last night. Not a big one but still a huge relief. My tummy has been feeling funny all night (like i've eaten something that doesn't agree wit me) So hopefully the extra water i have been drinking and stool softeners have been helping.

Going to keep on the improved diet, stool softeners and cream and hopefully this thing can heal now. If it gets bad again i will be going to the doctor for advice. Thankyou everyone for your help, never thought at 3am i would be so happy to go to the loo Blush
