How long until I know that otomize is not working?

I have dodgy ears anyway, they seem to struggle to dry and I get swimmers ear without actually swimming a few times a year. If I catch it early enough Otomize usually does the job and avoids antibiotics.

This bout feels different though and I feel like the spray is making things worse but I'm not sure if I'm imagining that as I'm in so much pain, should it be a fairly quick worker?

I've had the spray since thurs so I know not long but I'm feeling very irrational and fed up with lack of sleep, pain and wanting to chop my ear off.
