Worried about dh, bottom pain (TMI)

For about a week or so, dh has annoying achy pain around the anus.
He says it feels like he needs a poo, but he doesn't, not that he keeps trying, he is regular, no changes at all to bowel habits, just there's a feeling there.
He's assumed it's piles so has been using anusol, which has helped a little bit, but generally there's no improvement.

We are both worriers, his DM had bowel cancer. Does this sound like he should be worried?
Obviously he's going to see his GP, I'm not asking for a diagnosis over the Internet, just some reassurance if anyone has had similar.

Could it be internal piles? Sorry I don't know much about piles tbh.

I think it's always best to checked out just for peace of mind if anything else.
