Seeing doctor tomorrow about prolapse - what should I ask

Hello

For anyone with experience of this, would really appreciate your advice. Was diagnosed about 4 weeks back with a bladder prolapse. Given my symptoms I am pretty sure I have a rectocele as well, so booked in to the doctors for tomorrow to get that checked out too.

In my original appt I was given a link to a website with pelvic floor exercises, and told to get on with it for at least 6 months. No mention of what might be aggravating it, other options for me, just told to return in 6 months if there was no improvement.

My prolapse is mild, so I feel kind of guilty pushing, but would really like a bit more advice and help with it, as I am feeling very lost. Especially on things like how to lift my 2 year old (know I am not meant to, but that isn't an option), what exercises I can and can't do, whether I am doing the pelvic floors correctly. Basically I want to have the best chance of managing this so that I can put off an operation for as many years as possible and have another baby.

So from those who have experience, is there anything I should be asking for? What should I say to the doctor.

Any ideas really gratefully received, only just getting my head round it all, worried I will just have a massive mind blank at the doctors as I get very upset/ freaked out talking about it all. x

Thanks Cardamomginger. Well, got reassured that everything was mild and no anterior wall prolapse which is great. No physio which is fair enough, but might look into getting a couple of private sessions just to check I am on the right track. Got treated like a total timewaster which was a bit humiliating, but then so is the whole condition, so I am used to that! Will keep looking into what i can do for myself.

Sorry to hear you felt like you were timewasting, Tadpoley - and well done for not giving up.

Makes me cross - link to a website and do your exercises for six months and nothing else? No wonder women don't comply, that's not great care!

First of all - don't panic. There's a train of thought that says that if a woman's had two vaginal delivery it is normal for her to have a grade 2 prolapse. Does't mean she has to put up with it, just that it's essentially a normal consequence of birth because it's soooooo common.

Second of all - do yerblardyexercises. That really is key. If you do them, you'll notice a difference within 6 weeks. If you don't notice an improvement (not neccessarily a cure) by then - see your GP again.

So, what webpage did they direct you to?

I keep it simple - it's compliance that does the trick.

1. hold for 10 secs. Really squeeze - you should feel a "drop" when you let go
2. 10 quick flicks
3. Imagine you've got a tiny wee lift in your vag. Bear with me! Lift the elevator to the first, second and third floors and then back down again.

Do those three exercises at least three times a day. Every day. Without fail. Until you die.

Simples.

If it doens't help, then you can get assorted tools that will assist in doing the exercises, or give you feedback from doing the exercises. GP can prescribe some - like the pelvic toner that advertises on mn.

So, questions to ask are:

what sort of prolapse do you have?

What should you avoid doing?

What exercises should you be doing?

How much strength do you have in your pelvic floor? (ie, can you squeeze)

How long will you have to wait to see a women's health physio?

Will they prescribe you a pelvic toner?

Take a list of all your symptoms - any peeing/pooping/farting when you don't want to needs to be reported. Also, any loss of sexual function - so can you feel anything, and can you orgasm the way you want to? Any feelings of things "falling out" or bulges?

And, at the risk of sounding like your GP won't do her job - don't take no for an answer. Continence is not on the QoF scheme, so anecdotally, sometimes patients need to shout a bit louder than they'd like for a referral. THough, please don't shout at your GP!

Not sure I am brave enough to go back for a third time. In fairness the exercises have helped my symptoms a lot, but it does make me cross that neither doctor actually asked me what my symptoms are. And the second doctor told me I was fine lifting my 2 year old, even though doing so makes everything feel alarmingly bulgy.

I saw that you are a pelvic floor specialist physio. Assuming I know what type of prolapse it is (cystocele), would a women's health physio be able to help with the other questions like what I can't do, whether I am squeezing correctly, if I need a toner etc?

i'm with you there, tadpoley, it is all really sad - such little good information around for such a common ishoo. Huzzah for tinterwebs, and, if you use FB, have a look at APOPS group - there's a bunch of eminently qualified folk giving advice for free on there.

Lifting heavy weights will be a problem for you. i know, I know, I know - 2 year olds are heavy and wriggly...and there's not really a solution to it when you've got a resistant child needing to be shoehorned into a buggy or lifted into a car. "Avoid heavy lifting" ehm, ok. I understand that's not easy (ask me how...)

Yep, a woman's health physio would be able to do a proper assessment and give you specific advice about what you should and should not be doing. GP can refer, or have a look in your yellow pages or google. I might know someone in your area if you pm me where you are.

You might find an www.incostress.com helpful - it pushes the neck of your bladder back in place so reduces the stress on your pelvic floor. And, it makes you #doyerblardyexercises but you dont' need to get your knickers off, which you do with the other gadget, which is also good kit - but using it three times a day with a 2 year old in the mix can be tricky.

It is perfectly reasonable for you to go back to your GP and ask for a referral. Frankly, they'd prefer that than for you to leave it a few years and then them needing to send you to a urogynaecologist to stitch the whole shebang back in place...

I'm hoping you will see this gussiegrips! As you seem to have better info than anyone else I have encounteredso far. I am 5 weeks pp and was diagnosed with anterior prolapse last week and was sent out of the doctors skipping with relief as she told me it would repair itself within 3 months, because my hormones get back to normal.

Having spent every night on the internet researching I'm now sure this is incorrect. Can you shed any light. I've no idea how severe my prolapse is on a scale, but it is at the entrance of my vagina, making the hole look closed, and I am aware of the bulge ALL of the time. sad

Hope you're out there....I'm so worried....
