I think I'm ready to acknowlege my depression but what do I say to the GP?

So after nearly a year of being in denial about being depressed/thinking I can fix it myself I think I'm ready to admit defeat and see my GP. I haven't made the call yet but I think I can now, whereas before I knew I couldn't. The problem is I just don't know what to say. What do I say when he says 'so what can I do for you today?' I'm feeling like I'll chicken out again when I try to picture what I will say.

Just start by saying that you have been feeling really low for quite some time. Say that you have tried to get yourself better on your own but now you acknowledge that you need to find some help?
