Weaning off Sertraline, really struggling

I've been on Sertraline for over 3 years but it hasn't been working as well as it should have so I'm currently coming off it to go onto a new med.
I was on 200mg and I'm reducing it by 50mg a week which I thought was really fast but it's what the psychiatrist advised.
I'm now starting to feel really agitated, panicky, sick, and headachy. I've also been having intense/intrusive urges to self harm.
I've been having panic attacks again for no reason and feeling awful.
My moods have been flying all over the place.
They prescribed me 2mg diezamapam to take for anxiety but it isn't really doing much.
I don't know if I should phone the psychiatrist on Monday or just try to push through it.

I think phone the psychiatrist. Maybe go back up by 25mg for today and tomorrow til you speak to them
I found sertaline a bugger to come off, bizarrely the lower the dose the harder I found it.
