I have been told her to send off my CV to my district manager as a position has come up for promition.
How do I word the cover letter?
I want to put about work that I have done with my manager, do I mention her by name or just say I have done this with my manager?

There are a lot of samples on the web. Start with those and customise heavily. Don't mention manager by name - it isn't about her. It is about your achievements!

Do you have a job spec? If so, try and tailor the cover letter to that eg "Experience in knitting: I led the little snoring knitting group for three years"
