Today my partner took in a letter to give notice of paternity leave and his employer has told him to needs to get a form from the midwife, they did not ask for this last time he took paternity leave. What do they actually need? he has been there for 3 years and earns enouth to get SPP.

Thank you

I presume they want to see the MATB1 when you get it.

you get a certificate from the doctor, the one that gives you the free prescriptions and dental care.

I don't think it's that one Smelly - that's just the form you send off for the free prescription card. The employer will want the formal MATB1 I think.

When are you due? If your partner is entitled to the new Additional Paternity Leave coming in next year, it might be one of the forms mentioned here they mean. They require a statement from the mother.

Well, they don't specifically need to, but they're allowed to ask for a signed declaration from the employee that they have the right to take paternity leave, and some find it easier to ask to see the MATB1 instead. Explain you don't have it and ask what they need. They might mean the forms Flowery is referring to, or they have the right to see a copy of the birth certificate once the baby is born.
