No clinical benefits from remdesivir, study finds

Treating coronavirus patients with the antiviral drug remdesivir showed no “significant clinical benefits” in the first randomised trial of its kind, according to research released on Wednesday, AFP reports.

In a study among more than 200 COVID-19 patients in Wuhan, China, published in The Lancet, doctors found no positive effects of administering the drug compared with a control group of adults.

The findings were released after US pharmaceutical giant Gilead, which makes remdesivir, said a separate large-scale trial with the drug had showed positive results.

“Unfortunately, our trial found that while safe and adequately tolerated, remdesivir did not provide significant benefits over placebo,” said Bin Cao from China-Japan Friendship Hospital and Capital Medical University in China, who led the research.

“This is not the outcome we hoped for.”

I don’t know what happened with the Chinese study and why it failed.

This drug will be fast-tracked by the FDA and will be given to 100% of patients.

Another new standard of care in US is giving patients blood thinners to prevent clotting.

I believe the first reported negative trial was when the drug was given to seriously ill people

There was at the time a suggestion that giving people the drug early in the infection might give a different outcome

On the China study “ That study was stopped early because of difficulty in finding enough COVID-19 patients to enroll in China, the researchers noted. Given that that study didn't reach full enrollment, the results are "inconclusive," John David Norrie, a professor at the University of Edinburgh”

I've read that certain treatments are only effective if started earlier. One of our local GP practices was trialling I think the anti malarial being given in the initial stages (after trdring) to see if it improved outcomes.
It would make sense I think.

It’s to do with how early it’s administered.

I believe the side effects are nasty though, so might be difficult to gauge if it’s ‘worth it’ when the patient isn’t particularly ill.

Yes, in China study patients were given remdesivir 12 days after testing positive for Covid-19.

Medical trials are very complex and it's not surprising that two studies find different results. Under normal circumstances, several of these would be carried out, many double blind and randomised. Then the various studies would be assessed, accounting for any confounding factors (was treatment too late, may be ethnicity, co-morbidities/underlying factors etc) would be taken into account before a drug is actually approved. But these are not normal times so FDA approval doesn't mean as much as it normally would. But it's a piece of good news.

As for Gilead, it's been in existence for a long time. I suspect the name was chosen in reference to biblical reference of "balm of Gilead". A rare perfume or balm that was used for healing in ancient city of Gilead. There is even an African American hymn
