Summer's calculus or first signs of decay

My child is a competitive swimmer so stained teeth because of chlorine is a constant problem. Was brushing teeth this evening and looked at the back of the two front teeth and they are very stained.

Not sure if the pics are clear but can anyway tell if this is first signs of decay or 'just calculus'. Last hygiene appt was at the start of June and am assuming that they would clean every single tooth?

Dental check was in June too so surely this would have been spotted or they would have been more concerned about back teeth.

This sort of discoloration is common for the front teeth but have just noticed the back and am a bit concerned
