Hernia or Unbilical Granuloma

My 6 wo has a small red lump appearing from his belly button. The belly button has previously healed well and no issues until a few days ago. After doing some research on here. On doing some research on here I thought it was a Unbilical Granuloma but after reading another thread I am confused because he strains and pushes for a few hours a day which makes me think it could be a hernia as the threads suggested? Does anyone have any experience of this? I have just moved country 2 days ago away from the UK and no Dr sorted yet so not sure what to do next!

The hernia used to inflate when crying too and didnt go away for about a yr.
Not sure what other thing is though
