Health Visitor questionnaire Social / Emotional - Autism?

We had a health visitor today go through a questionnaire which I thought was called the ASQ. But now I look online for it, I can’t find it, and the ASQ seems much shorter and different.

It was based around social / emotional development for a 3 year old, and consisted of perhaps 20 or more questions, answered on a scale with responses such as Never, Sometimes, Always.

Apparently the range 0-60 denotes “White” and normal, 60-80 or thereabouts is “Grey” and warrants further investigation or observation, while over 80 is considered “Black” and warrants referral to Paediatrician.

Anyone know which test this is and where I can find more info on it please?
