Perhaps you should talk to your GP about all the reasons openly and let them advise you about what you should be signed off with?

General debillity was what a gp recommended when a friend didn't ant to tell her employer much. It's a very vague term.

Or exhaustion.

Most gps are very understanding, just discuss it with them and they will have faced this before.

The doctor can't sign you off with stress if you don't mention you are stressed! You could make out you a physically drained and simply unable to carry out your daily duties.

Worth mentioning, in our area any sickness with the potential to lead to long term absence and any mental health issues are immediately referred to occupational heath, cooperation and honesty are expected.

Here too (SW London) and not in a bad way - you need to be rehabilitated so that you can feel confident to go back to work.
Completely pointless to be 'signed off' without any appropriate medication or strategies to help you to recover. (I have seen many signed off that are left to stay under the duvet with no intervention to cure...)
Whatever it is that is wrong with you, OP you need the correct diagnosis, and treatment from a medical practitioner - they should not be asking you what to put on the form!
