Advice re dog being very subdued

We have an 8 year old Lhasa apso, last 2 weeks he's not been himself at all.
Will only go out to toilet , won't go for a walk, usually loves going out.
Couple of years ago he had same problem, had X-rays and they've shown a fused vertebrae.
He was shivery and appeared in pain, seen vets prescribed metacam, this has stopped the obvious discomfort.
Vet feels he has arthritis to hips/ lower back.
He has been having laser therapy to this area.
I've put lots of padding in his bed.
He appears really subdued, has his eyes shut all the time, keeps putting himself to bed.
When I look in his eyes there's no spark.
He is eating , not as much as usual but some.
Is there anything else I can do for him?
Hate seeing him so miserable, he's usually a really bouncy, happy little dog.

Well clearly you poor dog is in a great deal of pain. Have you made the vet aware of how bad the dog is?

I would go back to the vet and have a further conversation about how you and the vet are going to manage this level of pain, doesn't sound as if the Metacam is doing much to help.
