There is a small section at the end of the DLA form where you are expected to put in the name and address of your child's nursery or school.

I am reluctant to do this as my son has a mild and sort of invisible form of cerebral palsy(hemiplegia). What I mean is that it affects him a lot, but he is very competent in overcoming many of his hurdles. This gives the impression to those who don't watch him carefully that he is doing really well. These people have no idea, however, the work that goes in at home to getting him where he is.

I have all the professional reports that give an accurate description of my son's needs - but know that the nursery think he is fine and that if the DLA contacted them they would say so.

I've heard a few children with hemiplegia having problems when the DLA contact their school and their school gives a very positive report of their needs. They have been turned down after years of recieving DLA and there's been no going back.

If I leave the section blank will they question me?? Nursery is not mentioned any where else or in any of our professional reports. As nursery is not compulsory yet I hope they'll just think he stays at home - but maybe they'll look into it ...? I don't know.

I just want the process to go as smoothly as possible. Last time it took forever and was very distressing.

Does the nursery have a report of your DS needs..as this will help to make sure they follow the report,they will be able to understand ur son's needs.If and when they receive a call from the DLA...they will be able to give a good account.. How many days does ur DS attend nursery? who pays for ur son's nursery placement?..if it is the govt then you need to put it there as they have a way of finding out.
