dla mobility - how do i apply

Hi, my dd is 5 in NOv and has been awarded middle rate dla for care. I want to apply for mobility element when she is 5. How do you go about this?, do you have to reapply for everything again or just get reconsidered for mobility.???

Cant you apply for mobilty at the age of 3......? I could be wrong.

I wouldnt have thought you would have to re-apply for care as well but hopefully this bump will get some more helpful responses for you.

you can apply at 3 but I believe that you have to be getting high rate care to get it at 3. I applied when DS was 4 (the rules had just changed) and his awarded was changed from middle care to high rate and motability.

You can apply for high rate from 3 and low rate from 5.
I applied in May when DS turned 5. I phoned them up and they sent me a 'pack' with about 3 forms in it. I phoned the helpline as I was confused about which ones I needed to fill in, filled in the mobility on and didn't need to do the other 2.
Having recieved their decision this weekend seems that the form was read in complete isolation with no consideration for the things I have written about his lack of awareness of danger etc on the personal care form. So if I was filling it in again I would do so as though they had never recieved anything about him before though from a mobility angle.

The Cerebra website has a fantastic donwloadable form on how to fill out your DLA application. It gives you an idea of what sort of comments DLA are looking for. I found it really useful.

just found this thread and am so glad. Got DS2 renewal pack in and need help completing it.

We had to go to panel appeal last time and really dont want to do that again

fingers crossed it saves me from it this time
