Should I be concerned? Not sure about DS2

I've wondered about DS2 on and off since he was a toddler but he is well behaved at school so I've assumed there was no problem but he's now 12 and I'm not sure if there's anything wrong or I'm just an ineffectual parent. At times he can be the sweetest, most sensible boy but at others he flies off the handle easily and looses his temper.

I don't know whether having a label (if there is one) would even make a difference at this stage?

Are there any tools which would help me do an initial assessment of him?

To give you a few examples: He's always struggled to sleep and quite often would rather sleep on our bedroom floor than in his bed. If he does sleep in his bed he usually takes the sheet and duvet cover off as he doesn't like the feel and would rather just have a blanket (which he has chosen after feeling them all), he always wants to sit on my knee even in public, he talks none stop and can not sit or keep still, he's always fidgeting or bouncing about and if he is still he is tapping or hitting things, his concentration and short term memory are terrible, I can ask him to go and clean his teeth and by the time he's got to the next room he has forgotten.

Sorry for the long post, I'm just downloading! As I said I don't know if there's anything wrong or if it would even make a difference now but it's been worrying me for a while so thought I might ask....
